Intake Process & Needs Assessment – Louise House
At Louise House, our intake process is designed to ensure that each prospective resident is appropriately placed in a safe, supportive, and structured living environment. The goal is to understand individual needs and determine whether our program is the right fit to support long-term stability and well-being.
Intake Overview: Step 1
During intake, we gather essential information to better understand each individual’s background and current situation. This includes personal details, emergency contacts, health history, current medications, level of independence, housing history, and any relevant social or support needs.
Needs Assessment: Step 2
A comprehensive needs assessment is completed to identify the level of support required in key areas of daily living. This includes personal care, medical needs, mental and emotional health, social engagement, safety considerations, and overall support requirements to help residents maintain stability and independence.
Appropriateness Assessment: Step 3
We also conduct an appropriateness assessment to determine whether Louise House can effectively meet the individual’s needs. This includes evaluating the resident’s ability to participate in a structured communal environment, follow house expectations, and function safely within the level of care provided.
Admission Decision
Following the intake review, needs assessment, appropriateness assessment a determination is made regarding placement:
- Approved for Admission – The individual is a suitable fit for the program
- Approved with Conditions – Admission is possible with specific supports in place
- Not Appropriate for Admission – The individual’s needs exceed the level of care available at Louise House.
Our Commitment
Louise House is committed to ensuring every resident is placed in an environment that supports dignity, stability, and personal growth. Our intake process helps ensure that each placement is appropriate and that residents receive the right level of care and support from the beginning of their stay.
Rates
Price is per bed:
Rates begin at $900 for semiprivate room.
Private rooms begin at $1,800 per month.

Program Eligibility & Resident Requirements
Louise House is a residential housing program and is not a nursing home, assisted living facility, personal care home, or medical treatment center. Our program provides supportive housing in a home-like environment for individuals who are capable of managing their daily personal care needs independently.
To ensure the safety and appropriateness of our program, all residents must:
- Be able to respond appropriately to an emergency situation within five (5) minutes.
- Be independently mobile, whether through ambulation, a walker, cane, wheelchair, or power mobility device.
- Be continent of bowel and bladder.
- Be capable of performing activities of daily living with minimal assistance.
- Be able to safely function within a shared residential setting.
*DISCLAIMER: Louise House does not provide 24-hour medical care, skilled nursing services, continuous supervision. Residents requiring ongoing nursing care, extensive personal assistance, or medical monitoring may need a higher level of care than our program is designed to provide.*
